This is an exciting opportunity to work across Clinical Services, Quality and the Home Care Package Program. Flexible, person centered workplace with a great culture, and the flexibility to work full-time or part-time.

Together with the team, you will:

  • Improve the quality of life for people living at home
  • Ensure consumer care needs and lifestyle preferences are met
  • Provide clinical assessments and develop plans
  • Develop and maintain client support plans, ensuring positive outcomes
  • Regularly review support plans and adapt with clients changing requirements
  • Collaborate with clients, families, support staff and health professionals
  • Collaborate with clients, families, support staff and health professionals
  • Manage education across medication management for support staff
  • Build professional relationships with clients and their families

The position reports to the Home Care Manager – Sydney West. With an inspiring thought leader for a manager, you’ll feel optimistic about the future plan ahead. As the Registered Nurse for this region you will be key in the consumer-directed care space.

And you are…?

An intuitive and experienced Registered Nurse and a team player. You have a unique capacity for educating and influencing people. Your authentic passion for consumer directed care and building capacity in a great team of good people is the thing you’re most proud of.

The nitty gritty:

  • An AHPRA certified Registered Nurse
  • Previous experience in community care coordination – Home Care Packages
  • Able to demonstrate a passion for compliance, quality and education
  • A collaborator in knowledge sharing and proficient in Aged Care Quality Standards
  • A true people-person who listens deeply and compassionately, both to our elders and your team mates
  • Current drivers’ license
